Before providing independent contractor services for Tipsy Artist, please complete and return the documents below.

STEP 1 — Download Your Independent Contractor Agreement

Please read the entire agreement carefully.

STEP 2 — Complete & Sign Your Agreement

Please fill in, sign and date ALL areas highlighted in yellow.

Before submitting, please make sure:

☐ Your full name is included
☐ All highlighted areas are completed
☐ You have signed the agreement
☐ You have added all requested dates
☐ Your information is clear and legible

You may scan the completed agreement or take clear photos of the completed pages with your phone.

STEP 3 — Complete Your W-9

Independent contractors must also provide Tipsy Artist LLC with a completed IRS Form W-9 for our business and tax records.

Please complete and sign your W-9 and save it to your phone or computer so that it is ready to upload.

Please DO NOT text your W-9 to us.

Because your W-9 contains private tax information, we ask that you return your documents through WeTransfer instead. Please send to the following address via WeTransfer:  Tiffany@TipsyArtist.com 

STEP 4 — Send Your Documents Through WeTransfer

WeTransfer is free to use. You will need to create a free WeTransfer account before sending your documents.

Once both documents are ready:

1. Go to WeTransfer.com

2. Create a free WeTransfer account or sign in if you already have one.

3. Choose the option to send/transfer files.

4. Upload BOTH of your completed documents:

☐ Signed Tipsy Artist Independent Contractor Agreement
☐ Completed and signed W-9

5. Enter the Tipsy Artist email address provided below as the recipient.  Tiffany@TipsyArtist.com

6. In the message or title, please include:

Independent Contractor Documents — Your First & Last Name

7. Send your transfer.

WeTransfer may ask you to verify your email address as part of the process. Please complete any verification steps so your documents are successfully sent.

YOU'RE FINISHED! 🎨

Once your documents have been successfully sent through WeTransfer, there is nothing else you need to do unless we contact you for additional information.
